2023年4月7日 · Click the Ordinances link under the Membership tab, or click here to go to the Ordinances page in LCR. Select Baptism and Confirmation from the drop-down menu, enter the name of the child to be baptized and confirmed, and print the Baptism and Confirmation form.
This completed form should be submitted to the church office or via email to: [email protected] before Baptism. Please note, minors who do not pre-register for Baptism will not be able to be water baptized without a parent or legal guardian being present for the Baptism to sign this form.
